What is a grievance complaint?
A grievance is an official statement of a complaint over something believed to be wrong or unfair. Preferably, a grievance or complaint should be submitted within one week following the event or incident. It is highly recommended that any incident be reported as soon as possible.
What is a safety concern?
Anything you feel has the potential to cause harm to people, property or the environment.
Who can use this form?
Any Open Signal member, artist, contractor, volunteer, student, producer etc. who has a grievance or safety concern regarding our services, facilities or staff.
Who can see my answers to this form?
Information submitted on this form will be forwarded to the Open Signal Human Resources Department for use in determining if an investigation is necessary. This form does not track your user account or IP address. Everything submitted via this form will be kept confidential and on a need to know basis.
In the event that an investigation is necessary, details with removed confidential information (submitter name, address, etc.) will be shared with the Equity Committee or Safety Committee for review.
